Miscarriage = ”Abortion”

It is essential to understand that your doctor may use the word “abortion” to describe a miscarriage. This is the preferred medical terminology. Abortions can be induced/ elective, threatened, missed, inevitable, incomplete, septic, and complete.

  • Threatened abortion. There is bleeding, but the cervix is not open. Most threatened abortions continue as normal pregnancies and continue without more problems.

  • Missed abortion. The placental and embryonic tissues remain in the uterus, but the embryo has died or was never formed (blighted ovum.) Usually, there is no bleeding.

  • Inevitable abortion describes a miscarriage that can’t be avoided because the cervix is open and bleeding has begun..

  • Incomplete abortion occurs when a woman has passed pregnancy tissue, but some remains in the uterus.

  • Septic abortion. This condition happens when all of the tissue has not passed and becomes infected. It can be a life-threatening infection and requires immediate medical care (usually including D&C.)

  • Complete abortion means you have passed all the pregnancy tissues. It is common for miscarriages that happen before 12 weeks.

  • Induced/Elective abortion is the abortion procedure that must be performed in a specially licensed facility and follow strict legal guidelines established by the State.

Surgical Treatment Of Miscarriage (D&C)

In some cases, your doctor will recommend dilation and curettage (D&C,) which is a “surgical” evacuation of the uterine contents. Many women choose this option as it is the quickest and safest way to end a miscarriage. In a D&C, the doctor will dilate the cervix a small amount and then use suction and often a sharp instrument called a curette to scrape the sides of the uterus and remove all the retained tissue from the uterus. This procedure generally takes less than two minutes. Unfortunately, very few doctors can perform this procedure in the office. Your doctor will almost always perform D&C in an operating room under general anesthesia.

Significant costs are involved with hospital/surgicenter D&C. Even if you have great insurance, deductibles are rarely less than $1000. Time is also a significant factor. As with any surgery, you will spend many hours in the hospital. Often the procedure cannot be scheduled for several days after your doctor has discovered the miscarriage. Sometimes the doctor's office even has to get authorization (permission) from your insurance company to do the procedure! In addition, surgical procedures are almost always done around your doctor and the hospital's schedule, NOT yours. We can usually get you in for the procedure, which can be performed under conscious sedation or even local anesthesia, on the same day.

HOW WE CAN HELP

Because D&C is the preferred method for elective abortion, our doctor has performed thousands of D&Cs exclusively in the office. The procedure is carried out in the office under IV sedation, commonly referred to as “twilight” sleep. This procedure is significantly safer, less expensive, and faster than general anesthesia. The procedure requires only one visit and costs only $750, including the consultation with the doctor, ultrasound, and all medicines except Rhogam (rarely needed.) We can often schedule you the same day! The recovery period is usually limited to a few hours until the medication wears off, and follow-up is not usually required unless the patient desires it. With the local anesthesia option, you can even drive home yourself after a very short recovery/observation period and be back to normal activity as soon as you want.
